About the Event

Join Tyler’s Foundation for Wellness Discovery Day, an exciting, hands-on social emotional learning (SEL) experience for students and families! Students will rotate through fun, interactive SEL stations, build skills through play and creativity, and celebrate their completion with a goodie bag—plus families can enjoy light snacks and an end-of-event raffle.

LIABILITY WAIVER & PHOTO / VIDEO RELEASE

By registering a child for Wellness Discovery Day, a community-based Social Emotional Learning (SEL) event hosted by Tyler’s Foundation on Saturday, March 7, 2026, at Brookfield Elementary School, Oakland, CA, the parent or legal guardian agrees to the following:

Assumption of Risk

Participation in event activities, including but not limited to movement-based activities, group games, mindfulness exercises, and creative stations, involves inherent risks, including but not limited to physical injury, illness, or accident. I knowingly and voluntarily assume all such risks on behalf of my child and grant permission for my child to participate in this event.

Release of Liability

I hereby release, waive, and discharge Tyler’s Foundation, its officers, directors, employees, volunteers, contractors, partners, sponsors, and affiliates from any and all claims, liabilities, demands, or causes of action arising out of or related to my child’s participation in this event, including travel to and from the event, to the fullest extent permitted by law.

Medical Authorization

In the event of an emergency, I authorize Tyler’s Foundation and its representatives to seek emergency medical treatment for my child if I cannot be reached. I understand and agree that I am financially responsible for any medical care or treatment provided.

Photo & Video Release

I grant Tyler’s Foundation permission to photograph, record, and use my child’s image, likeness, voice, and participation in photographs, video, or other media captured during this event. These materials may be used for promotional, educational, or informational purposes, including websites, social media, newsletters, and printed materials, without compensation or further approval.

Code of Conduct

I understand that participants are expected to follow all instructions and safety guidelines provided by event staff and volunteers. Tyler’s Foundation reserves the right to remove any participant whose behavior is deemed unsafe or disruptive, without refund or liability.

Acknowledgment

By registering my child, I acknowledge that I have read, understand, and agree to the terms outlined in this Liability Waiver & Photo / Video Release.
